CANNELTON — Registration for Cannelton Elementary students will be from noon to 7 p.m. Monday, July 27 and from 9 a.m. to 2 p.m. Tuesday, July 28. Registration will be at the Myers Grade School cafeteria for preschool through fifth grade.
Tuesday, Aug. 4 is the first full student day of school. A parent or guardian is required to accompany any student registering. Students do not need to be present to register. All registering families must fill out a state textbook application form.
Payment of books and fees is expected at time of registration. The following information will be needed for registration: Social Security number, date of birth, birth certificate, parent-guardian employment numbers and three emergency numbers.
New students must have health and immunization records, birth certificate and previous schools' addresses.
All preschool through fifth-grade students must register before the first day of school even if students preregistered in the spring.
All preschoolers and kindergartners must have birth certificates and health and immunization records.
Kindergarten students must be 5 years old on or before Aug. 1, 2009, to be eligible to enter school. All kindergartners and any new first-graders must be complete or on schedule with their Hepatitis B shots and should also have received their second measles, mumps and rubella shots before they enter school.
Pre-schoolers should be 3 years old by Aug. 4, 2009, and must be toilet trained.
Breakfast and lunch programs will begin the first day of classes and are available to provide nutritionally balanced meals to all children.
The school prefers to have payment for the week's meals on the first day of each week and charging is not permitted. Students are asked to pay for their meals in advance either daily or weekly.
Children from households meeting the federal income guidelines are eligible for free meals or reduced-price meals for breakfast and lunch.
To apply, complete an application as soon as possible. Sign it and return it to the school office.
